We ask patients or families who have concerns, complaints, or recommendations for change to follow the process described below. These steps ensure patients with the opportunity to have their concerns rapidly addressed by staff members who are most familiar with their care.

Please speak with the members of your healthcare team first. Usually, they can quickly resolve most issues. If your healthcare team is unable to assist, please ask for a supervisor or manager. Our goal is to resolve your concern before you leave the hospital.

If you are not satisfied with the response you have received from your healthcare team, supervisor, or manager you may call our Risk Manager at (541) 269-8100. You also may send a written concern to: Risk Management Bay Area Hospital 1775 Thompson Road Coos Bay, OR 97420.

For complaints the organization recognizes as significant, you will be notified of receipt and notified of follow up.
